Complete this registration form and drop off or mail in along with the entry fee by October 15, 2010. Draw information will be available on October 20th at this website Entry fees are $50 per adult player per event and $25 per junior and include t-shirt, court time, kick off event, get acquainted Saturday Night Social and trophies for winners!

Tournament Details Court assignments and play times will be sent via e-mail. Assignments will also be posted on-line at www.christushealthfoundationsetx.org and at posted at the Friday Evening Kick-Off event. If neither method works for you, contact Stacey Ojemeni (409) 899-7185.

Friday Evening Kick-Off Players are asked to be at the Wilton P. Hebert Wellness Center on Friday for drills and clinics with local tennis pros from 6-9 p.m. Player assignments will be given at that time.

Saturday Night Social All players are invited to attend the social on the evening of the 23rd at the Wilton P. Hebert Wellness Center from 7-9 p.m. Enjoy this casual event with your fellow players. There will be a fast serve contest and you may also peruse the silent auction.

Playing Times – Details Play begins at 8 am Saturday and again at 9 am on Sunday. The tournament reserves the right to cancel any tournament event for low participation. Refunds will be given.
